If you are able to down load and complete the patient information packet, you will be able to expedite your check in process on the day of your appointment. If you cannot fill the forms out before your arrival, please allow 30 minutes to complete the necessary paperwork.
If you already have a diagnosis of breast cancer and are seeking a second opinion, please let the receptionist know this at the beginning of your call. We will make every attempt to schedule your appointment with one of our breast surgeons within 48 hours. Please be certain to obtain all of the necessary, films, reports and pathology slides for your evaluation. ( See what to bring to your appointment)
